Flu vaccine is mandatory for all children in New Jersey. Parents, take the time and make that call today to schedule an appointment with your child Pediatrician. Should your doctor advise otherwise, due to other health complications concerning your child, please request signed note from the doctor for the Day Care records. The note must contain information about the doctor’s business information such as: Business name (Logo), complete address, phone & fax numbers, child’s name, date of birth, reason for the visit, and doctor’s authentic signature.
Also, a parent can opt out of vaccination if you submit notarized written letter properly endorsed by the parent(s) on file for following reason: religious belief.
